REMINDERS, RECALLS AND TEST RESULTS
Kookora Surgery employs a computer-generated reminder system to inform you about upcoming immunisations or your regular Health Assessment, often through letters or phone calls.
We also assist with State or Territory reminders, such as PAP Test notifications for women. Following up on test results is vital, particularly when advised by our staff or your doctor.
If unsure, inquire at reception for guidance. Your proactive approach ensures effective healthcare communication and supports your well-being.
NEW PATIENTS
As a new patient, we kindly ask you to complete our ‘New Patient Registration’ form. This essential step enables us to gather the necessary information to maintain accurate health records for your care.
We encourage you to share details about your cultural background, aiding us in offering additional care or resources tailored to your needs. For your safety, we also recommend providing your preferred emergency contact information to your GP or our reception staff.

Are You Eligible for a FREE Flu Vaccine?
You could be eligible for a free flue vaccine if you:
- Are between 6 months to 5 years of age
- Are over 65 years of age
- Are Pregnant

Booking an extended appointment is important for more complex problems
Insurance medical, health review, counselling, a second opinion, etc. This may involve a longer wait. Please bring all relevant information.
If more than one person from your family wishes to see the doctor simultaneously, please ensure a separate appointment is made for each family member.
Please notify us if you are unable to attend an appointment. Fees may be charged for missed appointments – at the discretion of each Doctor.
REFERRALS
Your GP may refer you to a specialist, and this can be communicated to you through a written document handed to you, sent by post, emailed, or faxed as per your preference. In urgent situations, referrals may be expedited via telephone.
MEDICAL STUDENTS
We are pleased to contribute to the training of young doctors and may have a medical student occasionally present during your doctor’s consultation. Your consent will always be sought before a student can observe your consultation.
REPEAT PRESCRIPTIONS
Typically, your doctor is willing to provide a repeat script for your medication. To arrange this, patients are encouraged to schedule a phone telehealth appointment, ensuring they have the specific name and strength of the required medication on hand. However, an on-site appointment will be necessary if you haven’t had an appointment with one of our doctors in the last six months.
X-Rays
Patients now receive their X-rays directly after their Radiology appointments. Kindly remember to bring them with you to your follow-up appointment. Any X-ray films held at the Surgery will be retained for a period of 6 months only. Ensuring you collect them is your responsibility.
